Note: Badges are required for access to all educational
sessions, trade exhibits, special events and evening receptions.
CliQ 2011 authorized security will restrict those not wearing
badges from attending these events.

MCPF™ Examination US$400 for members/employees of members,
US$565 for non-members. Payable by check, money order, VISA,

Advance Registration Deadline: August 12,
2011.
Advance registration is available until August 12, 2011. Badge
packets are mailed prior to the convention for all advance
registrants, unless requested otherwise at the time of
registration. Due to possible mail delays, all requests received
after the advance registration closing date will be handled
on-site. PMA recommends that international registrants use credit
cards when submitting payment. Payments in U.S. funds only. See
details below for registration options. Payment by check or credit
card must accompany your registration. Make check payable to
PMA.
International Registrants
International registrants should plan to register by July 12 in
order to receive badge packets by mail prior to the convention.
After July 12, international registrants should plan to pick up all
of their registration materials on-site. To expedite normal
mailings, badge holders will not be mailed to registrants outside
the U.S. & Canada. You will receive a ticket to pick up
your holder on-site in Las Vegas. Registrants from Brazil
should plan to pick up ALL of their materials on-site.
Badge and Ticket Mailings Registration materials will be mailed
within two weeks prior to the convention to all registrants.
Requests received after August 12, 2011 may be handled
on-site.
For registrants outside of the United States, please return your
forms by July 12, 2011 to receive your badge by mail prior to the
convention. Or, complete the express mail option noted in the
payment section of this form.

Refund Policy
Convention registration and special events cancellations must be
received in writing by September 6, 2011. Refunds will not be
granted after this date. Send cancellation request to the PMA
Registration Department, 3000 Picture Place, Jackson, MI
49201.
